The THINC Knowledge Mobilization and Impact Hub (hereinafter called the THINC Impact Hub) concept stems from an evidence-informed and community-engaged funding design process that IHSPR used in the development of the overall THINC initiative. A key component of the THINC initiative, the Impact Hub will operate as a single, pan-Canadian hub and coordinating centre to build and support a vibrant, pan-Canadian learning community involving all THINC program grantees and knowledge user communities, including policymakers, administrators, health care providers, patient and public. The overarching aim of the Impact THINC Impact Hub will be to support and advance evidence-informed integrated care transformation and catalyze progress towards achieving the Quadruple Aim and health equity.

The objectives of the THINC Impact Hub are to:

  1. Foster coordination, shared learning, and collaboration across THINC grantees and knowledge user communities.
  2. Support capacity development in domains critical to the success and impact of THINC grantees and knowledge user communities.
  3. Lead initiative-wide KM activities to bolster the dissemination, translation and equitable uptake of evidence related to integrated care.
  4. Amplify and evaluate the collective impact of THINC projects towards achieving Quadruple Aim goals and health equity.

Relevant Research Areas

The THINC Impact Hub will lead, support and amplify the KM and impact of the broader initiative and its program components, including:

The THINC Impact Hub will incorporate six (6) core elements in its design and approach:

  1. Build and support a THINC learning community.
  2. Foster meaningful engagement among THINC stakeholder communities.
  3. Develop capacity in domain areas of relevance across the THINC initiative.
  4. Advance knowledge mobilization and evidence support.
  5. Amplify collective impact across the THINC initiative.
  6. Robust governance plan for Hub leadership and operations.
